standard letter for Niederlassungserlaubnis

Hello everybody,
I've been looking for a sort of standard letter to ask for Niederlassungserlaubnis (permanent residency) as high qualified personnel, but can't find any here. I found a very useful one from the supervisor but need also one for the applicant.
Does anyone can help me out on this?
I need it for this afternoon....
