Applying work permit for Germany from Pakistan : Help needed

Would like to know if any body have recently applied for work permit from Pakistan. I am planning to apply for work permit and have signed my contract with the company in germany. Looking for valuable suggestions for visa and have questions.
-- What documents I need to submit at embassy.
-- Do I need to have accommodation and health-insurance in germany before applying the visa.
